I think pretty much reflect some of the questions we've gotten from some of our callers. Red tile dysfunction. Which is a big issue in the talk about it. Viagra so -- the -- all those those other you know which is the best OneCare which is the vestiges right for you which is strange for you a -- best. You know large members and we got a number of calls on that alone and I and I just have to defer to them but talk about what is an orgasm how does it happen what does it do. Vasectomies prostate cancer prostate problems testicular cancer. The last question that was kind of -- can we cure wet panties. I was done it but after that I am I I think he must be referring to female stress urinary incontinence yes.

I things sneezing laughing or I love those new nuisance is that happen after you have children you know children can really do a number I don't woman's body. And we we probably see that many commitment almost the Miami a lot of my patients a lot of breast surgeries bloody dominant class. And -- in these guys get a lot of I'm I'm guessing a lot of patients from. From pregnancies and side effects of princes as wells aren't absolutely yeah. So please feel free to color to a 7666066172666060. With a plastic surgical we'll take -- because. I guess you probably should start. At the top. Was started by giving them -- either gonna talk about having an yeah I love that sentiment I think you you know give yourself something this holiday season if if children are. You know if you've reached your full complement give yourself the gift of those that to me -- I guess it's. -- your wife figures I guess as well as learn they that's kind of what more do you need to give her yeah that is so maybe you should talk about. What that's like because if enough manner. Scared the heck I. Really you know.

Does that to me. When the man has been through vasectomy you know at the end of that. Ten or fifteen minutes that it takes to do the sect may say most may get up and go that was so easy to sit that was easier than going to the dentist has now been.

Even have to open your mouth yeah. That's -- hoping that -- awake through it. -- the procedure act exactly do you under local anesthesia and yes it was a little sedation or something where -- Sedition really isn't required to FYI and local anesthesia takes care of everything and what about the recovery time. 24 to 48 hours watching television. And be waited on hand and foot that's usually about it networks that works for most people let me -- something -- in --

Let me ask the question I'm sure among may have a -- going to be asking. -- dysfunction. -- vitro -- Alice Viagra. So what are these things how they work do they work who should use them any the any about please.

Doctor -- All three are -- medications. You know all three of them two million purposes chart try to bring blood flow to the penis than an increase and an erection and -- in the erection. -- your limits are very similar site else's. The one that is a little bit different word -- do you see the bathtub. -- Warm water -- But sales loyalists does offer of some men more spontaneity and this was planning and and -- for the men who. You have social encounters more frequently I might feel a bit more of contagious and but you know they'll agree medications sometimes you know it's. You know it's almost possible which one may work better run. May have left side effects but until very great medication soon. He didn't end up eating. Achieving an erection.

So -- be increased blood flow to the -- which allows you to do you have an racial longer than you normally would or -- and actually if you weren't able to Havilland initially set.

Kind of that doesn't necessarily have view on the you have an erection more than you normally would usually -- or shall activities and and after and an ejaculation and orgasm on the Russians doesn't Bradley -- upside and right and you know go to the phases. But it did promotes more blood flow to the pianist. So that if that helps in achieving an erection incident either -- reserved for men who. Are able to attain a change in direction that -- appropriate data and then sometimes means we maintain maintain again. This is also further they. The middle aged guy who just. You know always a little -- game -- and these pills -- roll back the clock you know you're going from forty to eighteen all of a sudden and it's beautiful and I think that's what it does it does it do anything -- the increasing your desire or the veto or and that sort of stuff or is it just physically. You know increase in size I would say about Tim -- doesn't really -- silly to -- that's but psychologically if you know you're able to perform better yes and then that's a lot of great positive -- in my many more then no more likely to. To have social encounters an accident and had our confidence so you think they're all fairly similar. Load each -- of the -- sort of a time release that what you're saying it's sort of would have -- sizzle of a -- longer so it stays in the body they say about their six hours or so. Okay doesn't gonna rush Brothers two hours but it will be there to. Help with directions we're gonna go to the phones at this point timeline one we've got Sarah.

So welcome to the show with the doctors have.

Thinking so much undertaking my collagen. A quick question I was immediately I got X. It's hard -- which decided that it would be great idea -- I don't about it yet he's still very young. I thought and three are.

It's. -- follow you and got -- there's like eighteen Michelin.

Six patients to patent -- you anyway. I'm -- pink bag you know don't worry of course that was not -- a lot of Latin. Wow I take it out when I was not secretly twenty companies why do you mean and it's spelled. What's the little hole be out -- wondering if there like a plastic surgery solution can read it and somehow it still -- warm all Antonin.

Has it just not closed doors and just like an indentation willow Warren used to be a boy it was exactly.

I don't think I can put -- peace -- worry about.

So as an indentation. I mean this nothing coming out there you know having any discharge or anything.

That I -- but it could keep Blake a little like. --

If you need to have that -- to have that sort of it's small surgical revision though we should cut -- discard part of it kind of bring it together you'll have a small scar that's flat. But it won't be a deep hole like you have never you need to have a little surgical intervention discard inside the diamond -- so you know -- Luckily they are all yeah that's possible. All looked weird error it shouldn't shouldn't equate goodness humility that I mean I guess the bottom line is literally have now. Afraid and rightly have -- bothers you so you know you visit this lesson well plastic surgeon and later all these -- to do -- make it better that's. That's a bottom line. -- much for the call.

So that was interesting the -- what -- a -- is a huge -- your practices that something you deal -- a --

It is it is you know one thing I think we should mention is that as men age they're testosterone naturally decreases and sometimes. A little help in the hormone department will help bring things back. And that also makes the the drugs work better if they've Viagra sister Allyson movie trying to work better when you have a normal testosterone -- and that's something that people often overlooked -- under a little tired. -- do things that you feel a bit less known than men don't decrease in hormone than it is with women and you and applause he. But really you know they say some. Start a reserve of religion thirties and early thirties -- normal just also slowly decrease in -- Pont's mental so is there any solutions for women are there any minute medications on the horizon. To help women's libido. Testosterone. For women -- have slowly lower doses women benefit the women's libido is driven by testosterone. And believe it or not the the anatomy. The female sexual responses very similar to the -- response and and it involves the same hormones a lot of the same blood flow at the same nerve endings and even the G spot is so. Replicated by the prostate in the man so. Interest has graph and -- Mandela's government what that stands so our guy it's. -- and enjoy it lands there and about the mid anterior vagina yes so yeah and -- That is despise you know there is in women its interest in the got a couple of different kinds of orgasm you have collateral orgasm you have G -- orgasm. And for the man you can have orgasm of course from the foul us and you can also have additional stimulation from the prostate. So. Mountainous very very similar in both men and women.

Now does does -- or any other immense effect any of those other. Parts of the areas where he just simply the physical aspect of making the -- harder set kind of how it works.

It it doesn't work but that's you know at that. The fact that you have more blood in that area. Makes things work better idea and it actually. Is that feedback loop that it stimulates -- to veto it makes you feel more in charge more in control.

To interest -- and religious tenant -- caller we didn't want have been there but left question what are some things a man can do to prevent premature ejaculation.

Excellent question -- hasn't great question. You know Doris. Some medications though their. Usually there anti depressant and anti psychotic medications and they just have to work as a side effect to doing a judge elation. And then met in the past people -- talk about decent surprising. Agent's top collisions to try. To delay ejaculation but you know that's not a lot of fun to -- can't feel right having sex that's that's no that's no fun either. We know it's one of those things where. -- I believe it's it's more of two practice and retraining and reworking no wires that. People you're. Have been angry to have sex when there were much younger and you know a lot of men when the first step for sexual encounters ruthlessness with an estimation you know. But in a closet the world you know in a bathroom somewhere by herself and the main goal is just reach orgasm and just -- over with a quick right register early and that's it exactly and that's what I think most men when they have issues with premature ejaculation that some. It's it's a matter of -- retraining and we were ten. -- the wires and and I counsel patients about its practice.

Do we call the squeeze technique known in the squeeze technique is basically practiced masturbation where. Man brings himself to the point of inevitability and then squeezes the -- to hold off -- you know. Ultimate its calculation and orgasm. And then to start the process over again and then to reconnect that mind and body together and be able to have better control. That's step for the first step in treating premature ejaculation. Second step -- treated with medications such as and a half from now. Or how well future and other kinds of medications that you can treat to. Now on the horizon there is. A medication that does still undergoing FDA approval but it's -- available yet that there will be specifically targeting patients with premature ejaculation. Exxon. But -- not a force was not available yet that's.

That's why us a lot more than I thought oh let's take a break -- come back on the other shuttle gonna talk about dumb penile enlargement is -- number of questions on that. -- 617.

2666868617. 26660. ACC win by your calls -- doctors -- in this. Is changing your life doctor Russo is a board certified plastic surgeon who's been in practice for nearly twenty years to set up a consultation with doctor -- and his talented staff. Call 6179641440. His office awaits your call at 6179641440.

you tired of shading you I don't want to hair in unwanted places make today the day you do something about it. Call the office of doctor -- for laser hair removal. 6179641440.

Welcome back everyone -- to changing you know I found out just for yourself -- certified plastic surgeon. I'm here to take your calls without -- James blend into the free flow through urologists and -- hospital with him. And we've been talking about all sorts of things primarily -- how this functions which are premature declaration has established a little bit. Good thing you know large and we invite you to join us.

66172666860. That 6172666868. So. Doctors -- and then we've gotten calls recently. About you know larger and is there such a thing is there way to do it does it work is a dangerous.

I think it's a sham journalist has put it out there yeah it's not anything that sanctioned by the American neurological association. It's not something that you'll find performed in any legitimate practice. And it's the operation itself is a bad operation that can be fraught with a lot of complications. I mean this is the operation itself cuts a ligament -- basically. Lets you down yeah and may appear to gain an inch but it's not really there right. --

So there's a procedure and they were to have an erection. -- directions payments yes not a rigid pianist who -- to view. Workable so basically you think all these things should be avoided their shams and someone tell you this some real world they should run as fast as they can. That's right other way yeah and then the other issue is dearth of course people are using filler is of all sorts of different types whether it's natural fat or something artificial that's implanted under the skin and the effective usually. And something that's not very smooth that can be lumpy can be. Or disfiguring. The and once it's in them guessing it's hard to fix it harder. Get rid there's a problem there it can and can lead to severe infection can even lead to loss of the members so. Well so tuition we have knew what they have. Deal with it and use these other agents IBC. -- that Viagra or something like that could well be the best they can be. Lose some weight it's usually the -- that the that in your belly that's building up around all -- area that that's leading to. The appearance of shrinkage I -- it's not Tristan not actually shrunken and studies showing. Men think that is the age that things are getting shorter when they actually are not and that's been borne out in scientific studies. You're you stay the same link that you are home and the normal -- this somewhere between. Two and three inches in the placid stayed in five and six inches in the directs state that's that's normal that's and also can you do that against its two and three inches in the flash of state. And by the senseless is -- right yeah right so. You know most men are doing just fine the right there on the bell curve.

Yes but I have a -- that's existing and I had a feeling you gonna say that because I know we've had discussions about it and every time I mentioned you're like oh my god you go to roll your eyes were really. Three dollars now just think. I'm I'm in my fifties is that as I think you are and you know as many as men age their prostate enlarges right just kind of naturally. The most -- it just testosterone makes the prostate grow. And so eager to listen to get to a certain point where. You have to start getting up by the bed at night to pick. -- and you find yourself. With smaller amounts coming up from at what point does a guy go to someone like you start doctor -- for treatment and evaluation of this problem.

I was anytime live. Patio of the above the -- more frequencies daytime or night and it has become a bother it's -- interrupting their lifestyle and they're most. More. -- sleep you're there in daytime minutes into interruption of their lifestyle and that's something in the bothered by -- they should seek. They have positioned for for counseling and possible treatment. -- if they see if it's painful -- they see blood certainly go to the doctor right away sure if it's just an annoyance when it becomes troublesome that come on man. Yeah we have ways to treat this.

So though lots of medical medicine people could take us to take out how to shrink the prostate vacant interferes with a testosterone -- to -- well there's medication and there are two different kinds of medication when it's called alpha blocker in the third. Hypertensive medication that can be used for the prostate to actually relax the smooth muscle the prostate. There's medicine to shrink the prostate. -- we have somebody in the office to microwave the prostate is just provide some some energy to the prostate tissues and it's. Thirty to forty minute procedure that shrinks the prostate and then you don't have to take medication while. And it's with a laser roses the -- her with a green letter Lou what is with. We have we have two different kinds of energy microwave energy and radio frequency and Tuesday. As discussed -- like you read ended WRKO as well as some radio frequency -- to -- I assume it's paying less. Very well tolerated here no pain isn't really something emotional complain about doing the procedure in fact some men actually gonna take a -- simple sleep during the procedure. And it leaves you able to have a more normal life tonight after your name is -- kind -- shrinks the past a little bit. -- nine out of ten men respond very well to this procedure went out of ten men and basically unchanged yes. And most men than the nine out of ten -- usually thrilled because they have a better flow of the you know blessed night in a much better control during the day. They have less frequency less urgency. And it doesn't seem to affect you a lot of the medications and the gold standard surgery which we call TU RP trans jury through section of the prostate. That leads to retrograde ejaculation in this procedure doesn't seem to lead to turn to protect them.

excellent 72666868. At 6172666860. Were talking about plastic surgery and today. You're trying to urology because you were privileged to have doctors -- and clock here who work. Urologist at Milton hospital where -- on the same stuff we assume quite often they are their buddies a month. Passages excellent what they do and we invite you to -- our excellent -- can really terrific thank you very much. They have offices in. Milton of course kingdom in Boston and it can be reached at 6172962222. That's excellent seven. 2962222. They were in the same practicing you can call that number get other one of them. -- sector means how they reversible and what's this.

If and if so what's his successor. They can be reversible -- success rates can -- And -- times are factors that player Roland and percentage of sort of success after reversal. Usually. Time frame of how long has it been since the vasectomy. Usually further out from years of of having a vasectomy then the chance of reversal can be. Much much much more diminished interior bid that's within the first seven years yeah that's a news you got your best chance or sons.

Then return a sperm return to 90% now if you look at all the series and literature now it's gonna very you know individually. According to the patient. According to the situation at hand but it can be very very successful. Com but there's no guarantees yeah -- you know but it is the cheapest form. Infertility treatment as the sector reversal -- McCain now. Com and certainly much easier on the woman's right thanks Fisher. Com too big topics in your field of prostate cancer and testicular cancer. On what should our listeners of their know about that one of the warning signs symptoms what are things that people should do should we do exams attention please. -- prostate cancer and there are no warnings science and it's important to get an exam every year. Men starting at age thirty should really go to there doctor wow that's for baseline PSA at 1830. Of course. Now the American neurological association says everybody gets a PSA baseline age forty and I think. We've seen enough men with prostate cancer in their thirties to say that really I get that checked out early on. And a digital rectal exam every year and we follow the PSA year to year shouldn't go up more than three quarters of a point a year. Zero point 75. And there's also age ranges so if you're below fifty your PSA should be less than 2.5. If you're fifty to sixty your PSA can be 3.5. This year sixty to seventy year PSA can be four point five. And if you're over seventy your PSA can be six. As a baseline and however. It's all. Relevant to your individual situation that's only on your first PSA have to follow -- here and have to make sure your prostate has no abnormalities. And you need to make sure there's no infection or other mitigating factors that are affecting your PSA.

So it went to -- goes to their regular medical doctor they should be getting the rectal exam every year if you're not. Do that they should go there do you guys because you Hampshire I'm guessing on what really guessing but I know these guys are probably more expert at picking up. Small modules in the prostate them perhaps the average. Practitioner who you know isn't in -- urologist -- is an expert in these areas. -- correlated to a higher level of survival earlier treatment I'm guessing from well we have educated fingers we will allow the process feel like yeah. Now what about testicular cancer.

Testicular cancer really. Similar to women men and breast cancer mean. Young men should be counseled that. Once a month at least we shouldn't -- know their bodies and feel their testicles on a regular basis. Typically. You know warm situation in the shower or no bad thing. And just feel for Renault and he lumps or bumps or anything that doesn't belong there and protesting over you know should feel you know. All the way in almost like like a hard boiled experts say nice and smooth and its and its lumps or bumps. By no means they should seek medical attention as well.

As I remember testicular cancer was fairly treatable. This about -- were fairly -- from my my learning about the twenty years ago -- still okay -- Yeah has then testicular cancer. Has sort of been one of the superstars in the uncle logic world. And it's very responsive to chemotherapy there's a form Seminole and that's responsive to radiation it's a very very treatable anybody who discovers. And the abnormality on their testicles on as doctor -- says -- monthly self examination. Just like -- women with breast examination. They should seek medical attention they should see their doctor right away and it's very very treatable. Especially if it's picked up --

Where a 6172666868. Were talking with doctor -- and doctor -- from Milton hospital. Oh we've just gotten another question. What age should demand gets tested for testicular cancer -- don't know if -- test there is no tested to its it's an examining that I feel as he found the lump right.

It's no blood tests or. I mean dark X rays but usually the first. Signed an indication is really a mass. Usually it's firm and irregular feeling it's on the testicle and and though this doesn't belong there yeah. Yeah and that's usually with the physical exam where somebody as a lump they're gonna come and we'll get an ultrasound right on the ultrasound is sort of the gold standard tests don't Telesis they're mass in the testicle if there is. Then that's something that needs to be addressed if that NASA's outside the testicle fits on one of the other structures surrounding the testicle. It may be an entirely benign thing and it usually ends.

Not as I remember and -- against it changes you know ancient history for me but. You that's always floated as a young man's disease is that true or not true I can happen at any any age but there are a lot of them found no matter -- at just --

Sticking something wrong. -- just a different types of testicular cancer found at different ages ago. The younger men tend to have germ cell tumors were called -- tumors that are arising from things that produce sperm. The older men tend to have you know men in their sixties and about -- at things like lymphoma in the testicle and then men in their ages between. Forty and sixty would be the seminal moments.

Right press so I didn't he just really be vigilant and every age. An incentive self exam and certainly doctor should be examining new as well. And doing testicular exam when you go through yearly physicals are well.

Read those -- only related.

breast cancer is for women.

Well that's an interesting question that you think of prostate cancer is being the hormone only related. But a bit but there are different things you know. We different factors we look for them to things beta eight CG. And -- protein which can be produced by testicular cancer cells. And you contest for those in the blood so in terms of those sorts of things you know there is a relation. But they're not. Related to testosterone percent.

Or six when 7666060. We -- you. To join us -- there we talked about men's that the guy I was talking to you today at a hospital I was actually shocked -- mean thing when you first all this that half of your pace were women. And I ask this because I mean really a muscles are tightening up not a high single or how can that in the I think you said -- stones. And cancer and she urinary stress incontinence -- and you know we mean -- is just a contest I'm guessing for the most part is related to change that happened during pregnancy within the kind of stretched out and and age and you. We see age gravity in childbirth priceless and I know plastic surgery -- we have the same three things happening throughout independent child birth not really generate most of my business. And and so there's some length and very urologist plastic surgery lengthy and -- this out early remain have a you have compounds are triumvirate they do so because military in Tehran let's talk a little bit about urinary stress incontinence because that's a huge compliment isn't it in and how off how frequently do you see that hobby is it are the treatments. Six successful.

We we see very often we have excellent treatment for a we have many -- and the treatments evolved. Over the past twenty years and it's become very very elegant and minimally invasive. And you know all stress incontinence. Starts out is trying to do -- sectors exercises because you can actually strengthen the muscles in the pelvis and helped lift things up but at some point things serve have fallen down beyond repair here and they need to be lifted back up surgically and we have. Minimally invasive ways to correct that.

Com or -- on masses but sometimes so when my boys have one that's undecided testicles and it's not brought down properly or it's still. Higher writing it to appeal to help pay. A lot of times the the recommendation is to remove that testicle. The concern is that sometimes if it's difficult to feel and examine a testicle. I -- o'clock Birmingham -- and you know. I'm seventy years old but there are few years ago. My PS -- with everything about 45 and I do have a little white -- I think going. Quote for quite some time it hasn't had any effect I'd write. Formats that have had had no effect so I still have the same symptoms however I wanted to say that what my heroes just yet. -- found that might yet they would in the twenties and went from about back to -- want to know and I didn't believe that helped -- went to another book. -- And he found that it was me in nineteen. So I insisted that have a I ought to know I have a biopsy that was. Very very painful I wouldn't GoDaddy and that it turned out I did not have yet. So how do you explain this -- positive for. Would've been thought.

Well you bring up a couple of excellent points help. One is of people who were armed pro scar will have a PSA -- 50% of the normal PSA associate abusive fortify -- pro scar your PSA is actually eight to ten. And that's an elevated PSA. So it -- reasons that PSA can be elevated in somebody -- seven years old are number one enlarged prostate just a big prostate makes more PSA number two. Infection and inflammation and number three cancer. So you're urologist at the correct thing even putting you through the biopsy which can be uncomfortable or painful but doesn't have to be necessarily. And by biopsy new ruling out prostate cancer in those biopsy specimens. It doesn't rule out entirely that there could be prostate cancer in other areas of the prostate which warrant biopsies because you only sample certain areas. So it's in -- so it's a very important that you follow up with your doctor. That if they found inflammation that you can treat the inflammation and get back to baseline PSA. Which may be back to left fortify range after some antibiotics. The main point follow up with your urologist. Thank you --
