There was a lot of interest in Botox and -- others and breast augmentation and liposuction. And will both get into that a little bit. It was a little daunting initially to do a radio show I have really had no experience in radio up into the outside the -- and DJ in my college radio station back to question anything he actually. You have always said that -- mean you're missing -- fears and sort of funny how we ended up on the radio business isn't it I'm not. And I must say that I've really enjoyed it it's been kind of a learning experience for me. Both from the technicals plenty of actually how radio show works it's been fascinated but also. It's been interesting to hear what people are interested in what callers interest and in the kind of things to go through their minds and you know that the other thing is I guess doing this every day we take for granted how much information is out there and how little information people really have access to.

My eye surgery completely I am surprised all the time. When people come in an ask about things that for us is almost passe at this point in the aesthetic field and.

A lot of things we just take for granted that everyone would know we don't silicon versus saline implants everywhere and we know the difference between Botox and -- some very basic and they're there really isn't a lot of understanding about some of these things. So I feel it's -- elect our job to. First of all demystify some of the midst debunked some of the stuff that -- people for example asked me almost a daily basis. Isn't it true that after ten years after changed my breast implants. Now I can't tell you how many times have had that question. And it's always exactly the same question I don't know where -- came from totally untrue.

I even get it as well I navy has to do with the warranty that people think that -- in ten years my warranties gonna run out selling. I hadn't thought of that replace it.

That might be -- but he'll people who commute for the very first are people who even had been for a long -- I'm almost in my tenure marked a -- a month conflict what for a lot. Only other tenure thing but -- things like having your tires are executed a close look at -- so so if the show has -- taught me the little bit about the fact that we really need to execute people some very basic information -- about what is we do. And I'm trying to think the best way to relate that the -- thinking one of the ways he might wanna do is just to talk about some real life examples of patients that we've treated. Over the years and how we've made tremendous changes in their lives. I'm thinking of a number different patients who went represent different she's redundant or get into that this was at but I thought that would be. Helpful because you'd consider maybe see themselves the in the example that we're gonna give about how patient came in with you know. Being embarrassed about how they look in some way and how we were able to help them through and it became you know different better person functioning at a higher level feeling better about themselves you know. We and countless examples. That we can relate to of this.

Last week I was telling about the article I read that one of the biggest things that they referred to -- everybody self conscious about something. -- when he addressed differently when you act differently. Most people are hiding some flaw -- they don't always wanna admit to. And it's baby steps to get people to finally address that. No less at least in a forum like this -- calling -- ask a few things that they've been curious about knowing is that something that's even commoner -- do you deal with this type of problem. Because a lot of times it's small things that are really bothering people.

Yes that's that's an interesting comment because you know I think almost everyone out there has something about themselves that they feel a little insecure about -- or. Who rejected PM. MM with one example of a patient that came in one time. And -- I you know looked at her she was you know very thin very Catholic and features like 58556. Maybe 129. You know very -- very -- very nice to -- could she be here for. And the idea I hate to talk to and examined -- her history and she said dumb -- she was interested in improving the apparent effort caps. Really. I thought oh well if you must have built the -- as a very -- must have some pretty thick -- that she -- all exposed to cast and I thought. -- of beautiful where they teach you want and bigger or smaller. Well that's why that was my first question I thought well maybe they're too since her what she wants it typically of people and calf implants we've done those kinds of things I thought while. So I'm thinking well you know most people would love to end up. And to myself of course and and -- you know wouldn't have William a very thin person number whatever I said -- 5612250. When there was. And my cancer just like bigger than I want from the B and they just above all never wear short and it's a very attractive -- I'll never wear short pants. On their go to the beach on the everywhere bathing suit Lama has an estimate that my calves and to a total sort of almost embarrassed to let me see them. I thought why now I mean how weird is that I mean these are Beazer.

Crew that catalog all those girls I'm all find myself looking through that catalogue and in seeing that they're all in shorts -- that -- these nice lawn theme legs particularly their cats in and when you look at standards like that out there I can see where she might. We'll have that sense of you know feeling inadequate it.

many of these at the set of possible standards for women and mean and making them a closed where they have a muffin top with a in this skin -- sticks out over the pants in these very very low riding pants which -- to a good and yes that's. Today in Edmonton in exposing cast expose -- body parts that there are awfully flattering for a lot of people who wear and so I mean. The standards and said it's such a high level that most woman can never reached them. But this particular element and this is probably old past ten years ago I mean -- happened you know -- still remember this gift it was distressing to me. Com. Source and it was in Africa which you can you Catholic really fine to me and now I don't think you can do a lot with them and she's. Yeah we had this discussion and and I examined there. And it turned out most of her calf was muscular she ran you know as a tennis player yeah she's very active you very much if and it was a little bit fat and everyone has a little bit defendant can and some people more than others. -- let me be very honest with you. You know if you do liposuction. Under Castro is the only procedure you could consider. You'll have some improvement but now it's ten to 15%. Immediate the most one -- yet that the very you know top line Tony. You have to ask yourself is it worth it is it worth spending the money. Where in the compression garments keeping your legs elevated not exercising for -- go through this process of surgery in the post operative care. Protect somewhere between 1015 -- need -- 20% improvement. You know at the most at the most. And in fact most people who see you will notice no difference between you probably will notice some but most people probably wouldn't notice and and you have to ask yourself is that we're spending this time money energy effort to do. You know I want you are confident and sort of under promise them in the fight over deliver or hit it right where I say I -- Then economic here oh they're great everything's wonderful world -- placed a limit. If I if I sort of tell people like -- and I can't and I don't. Do it then I'm got a problem -- excited to. Be part of our business yet and you -- beach dream and honest with patients in this business and I think. Some of the pages I've seen from other surgeons who you know had unhappy results have primarily been. From the point of view patients having unrealistic expectations whether the doctor just didn't spend time telling them what was possible. Or told them that they would really decent and they couldn't they thought they could you know -- but that's that's probably one of the greatest sources of litigation and in disciplined for patients. Is that they think they're giving him more than it.

This goes back to the other side of the coin which is people not knowing what they should do the people and NIC -- Botox or filler. They think they know what they're coming in for but what I see might be very different in some of it is explained no you don't do Botox. I -- cheeks it's more for crow's feet muscle movement things. Between the brows forehead. They themselves aren't always clear and -- in San -- I feel like I need to fill the lines between my nose in my mountain when really the issues they may really need something in their cheeks so. It's a lot a combination of setting expectations but educating them as well.

Yes the other part of the show I think it's important to media -- to you -- I feel an educational responsibility to our -- I feel like. You know this is you know you are really paying tuition but you know this is a chance to learn something new announced talking about the Senate.

I think we'll enjoy this part -- yet been -- day talking with people all day long in a lot of what we'd like to do is teach them. How to be. But a good consumers so that they know that going in nine -- even -- with the clients that I see if he should. Move to another part of the country these are the things you wanna know before you go and find someone to. Do surgery and you you wanna make sure he's credentialed if you're looking to do Botox -- make sure that. On gas something she now is this how many units -- can get things that are simple TO NI that they don't I we have to educate them.

It's gonna finish my story because they think about the pace with the level sectional because I think it it brings him an additional point. So we ended up doing that the surgery and as you know when you do that section of the lower extremities that takes a while. To see the final result because. Smiling yeah it's -- and -- cavities were against you this every time you stand up you know the food -- rushes to your -- your ankles and -- That's the one area where would take the absolute longest period of time to hear results where you have to. Actually you know keep relate to elevate it wears compression garment that we in the -- a special stocking compress the stockings to help accomplished put up. And you know -- I was happy that I had done the best job they could but I also knew that it wasn't going to be a tremendous -- difference because she didn't have Richmond terms Arafat to begin with. So I guess about six or eight months later you know when -- second third follow -- visits came in. And she came and I believe wearing short term or those knickers or some some some way to relate to it was a very. Exciting news I knew how big a deal that was for her. And she came in and and she was extremely happy. And you know she. Acknowledged that. She was certainly happy and and she wished they could have been a little thinner. And but she should but she knew that we had and victims she was really happy she had gained more confidence. And I -- the only way that relates to -- offenders who stopped exercise and at the muscle after you know and in the I mean that's the only OK it's it to make them fit because the only thing left was bone muscle. But that the moral stories that. You here's somebody that normally would be looked upon by society is very attractive woman who would have almost a perfect body but. In her own mind if she had this I don't think it's a terrible flaw which she had a a flaw -- miracles made -- uncomfortable about yourself parents obviously enough that she dressed. Yet around it yes and now through this miracle of surgery. She was not able to live her life more comfortable into the -- decades -- this is as -- as is can -- thank -- for you know all you've -- and I can move -- and from you know that's a small case -- point. Of of the kind of things we do but it but it stuck with meets up with with me in my mind in -- because. He changes woman's life. Yes you know it really did. And this was the only chance she could could have to do that. Nothing else was gonna make a difference for her the clothing and wear clothing over the rapidly nothing. Moving a little bit of fat thing you know this is all we can do all anybody can do. Yep and and to -- the move forward and felt good about it so.

Well that's kind of like the other patient that we saw that had the nipple issue. And the week you and I've talked about that in in maybe you can fill them in a little bit more on the details of it but.

Yeah I mean this the had a patient that that it -- If you've been a piece of -- for almost ten years and quite awhile with the number procedures on Hersh you've been other positions but she came to our practice. Slightly dissatisfied with punish certain she's seen and we've done a number different small precision her over the course of eight or ten years. And and exactly how the the the conversation starter but she came in and said that. One of the things that it should at a -- reduction and one of things didn't bother her word nobles were never able to get direct in their doubles are never able to two beasts seen through clothing and this is this something she. Really lamented the loss I mean she really felt like less than a woman I think it inhibited her sexually with -- husband. Even mannequins technicals he's -- and she didn't you know and it was sort of like. Com something that was taken from her and never given back and it was an important part -- sexuality in. You see. You know started discussing it with us and it was a little -- She'd never -- I think in ten years you know all the sudden again I don't remember what start of the conversation but certainly. Once he started into she felt very comfortable talking about it. And we talked to her about different options she had for improving her appearance of an apples and she wanted soreness in pianist can erect position all the time or at least the -- to where they. It would be seen with certain clothing and stuff and admitted to still more feminine you know. Com. And we went to a discussion and we ended a talking about putting feelers into the best into the nipple and found yet you came up with the -- he discovered since you came up with did I took credit for but you actually. Eventually came out for the just like an office has been confessing a -- that's -- on the radio. Yes well your ideas only.

Well he tried the -- first in that just didn't nine if it wasn't. Heavy enough to create enough of a patrician -- there was a lot of cartoon editor that restrict cell I said why she used to a product called it's -- Form of cortex that she can actually cut in shape. And he uses it sometimes in noses in and said wanted to create like a little implant we can just put it in the nipple and it well. Stick out. Her acclaimed and so she was open to the idea and you know all of a sudden he's like yeah let's traffic in -- creep in like little bullet yeah. And that was the beginning of on an office procedure.

That was fun through because it was a first whether -- and -- been practiced almost twenty years and I got a chance to do so they had never done before I had to think about had a car of myself -- be creative about it I had to think about how to put it in where to put it how to hold until late in place. I have a -- the -- and thousands thousand cases but I never done them insists different than the small things. But she actually left office crying not in pain not out of being -- she was so deleted. That's just the mere thought that this could be in imprudent for her just deposit she could have this nipple appearances she was -- she became -- to talk to about pension.

She gives you this really com. Taken back that it's for something that I think she'd been thinking about for a long time that some -- willing to listen to her and saying let's get this high you know we weren't opposed to we didn't. Say -- no you can't do that that's silly. We really made an effort to and we got -- right into the schedule and on happily she was really pleased with the results.

Yeah and that's a notification -- something again that. Most people might say Q what's the difference what you. But kind -- was critical -- this woman was crying about mean inhibited her sexually with -- husband you know something she was just not comfortable within. You know it's such a personal. Business that we do you know. You know you look at some NCG they have got to find looking whatever but to them -- not then.

It's not with Nixon -- there's also a lot of hidden things. On how many women have we seen commend that where two -- then a -- over a bronze and create the appearance of having a lot of breast tissue.

Right right. For some reason we mention I -- a woman -- they -- in two pairs of pants no one -- that. Doesn't apparently -- I didn't ask Diana did nearly one know what that's like -- is going -- physical. The and it has seen not as she. That's I thought -- let. Let's get one more example of of a life changing surgery that that would also that -- shift gears and kind of make you shift switch hats on -- interview you. So the listeners get to know you a little better. In about what you do and how you doin' some of the some of the questions that come up in your practice. One of the only has one last example of -- with the -- which I think. Tim knee is one of my favorite operations primarily because of -- give you the story I think you'll see eleven it's the most. Satisfying and gratifying from that think it changes lives to probably the greatest degree of any of our surgeries to reproduce in predictably reproduce ability. Affinity and I just saw a patient before I came over. Yes yesterday who. And it was actually flat as a board and had. These beautiful silicon gel implants put in and -- his beaming now. And she was talking about just how her life has changed in terms of she she's an Islamic patient. As soon as to what seven -- into its last now the reason she -- seventy squish fish looks so good in bid if it's now. If you -- before I'll be afraid to wear a bathing suit had the funds had the most padding that didn't reveal the fact that I was really flat and -- gave me some sense of having breast tissue well.

Being a woman. I can and thoroughly relate to that dilemma I would say more women and not have that problem can really do feel. On that they're not that famine in that you know every ad that you see out there every. Person in Hollywood is voluptuous and falling out of there bathing suits and for a lot of people it's a real tough. On fine to go out to be exposed in a bathing suit in not having to show for.

Hey welcome back and happy fourth of July of this introduction just official and Kathy stereo this is changing your -- from Boston's talked stationed. In succeed WRKO. Who are making history and we're talking about all things plastic surgery. If you missed the first part of the show shame on you but we've had a great discussion about what we do how we do it how blessed to you can change eleven elegant one more example. We'll pick a -- and talked Kathy. I'm thinking this one go initially and Marcia was seventeen initialed. This is probably 1012 years ago. She. And her mother. Came in for breast augmentation think that this was -- graduation present for her daughter from my school. This girl was seventeen was incredibly shy hardly ever spoke were very loose baggy clothing.

You know there's just kind of there but really had no significant personality you know would answer from yes now. Nice looking though pleasant nice family but just something not quite there she wanted to come not just mom. Here. In hand as it turned out she was on anti depressants because she. There's very flat -- and felt very uncomfortable didn't bill -- uncontrolled trying to have a boyfriend. Was just very kind of -- definitely I have the fact that she was sort of flat chest.

The parents appealed to calm.

The you know an insurance agencies to have discovered because it's psychological distress in the civilian that never works. So. They decided they would have answered on the mother because she wanted to do -- was to support her daughter through this. So the daughter has the surgery done. And mom -- on the same day and they kind of recover his mom also -- to. The -- was not flat chested she was a former Playboy bunny all from. Such a hundred years -- Com. And but -- she didn't have big -- mean she didn't she was probably its. B cup pressed the issue is old enough their best to -- smaller she's probably in her forties at that time mom. So she has lost some violent she was maybe a small -- need be she wasn't. And she did -- just wants something to reestablish which it lost you know maybe for the small state. And -- daughter was probably in the same range. So she did the surgery and you know didn't think much about it and then you know she came back from her post up -- visit three. For some the number of months later. And it was almost like someone flipped a switch in nations like you know -- switched her body for someone out there like that moving within. Body snatchers. She came in issues very local issues very pocketed she. And it was very outgoing was wearing sort of more revealing clothing for her she'd gone college got a boyfriend you know it was like this is the same little seventeen year old girl that -- many of social I've barely could talk to me. Now she's wearing this is -- more revealing clothing she.

more adequate and basically if she had a mom that had history. On being someone that she was measuring herself by piano you know like -- winner will be really difficult.

And I have always remembered that case because it was such a stark contrast it was such an incredible change. From the way she she started com will take a caller we've got Jessica. You don't doctor Russo WRKO you're on the air.

Thank you Jessica thank you for calling. And I'm. Actually about. -- About. 98 event at -- that have made that happen and popped am I and I did and now at. At -- accurate and I -- and at at Cox neck and did -- did it even -- that back on reality an Angel at.

Well that's an excellent question. In the answer is it depends. There are several types of implants most commonly saline -- and the other kind of silicon. That's sailing is salt water which is what our bodies may -- so. And if you've ever been in a hospital and you've -- a bag of IV fluid that's -- almost every patients stretcher and hospital room. That's the same stuff we use to put into breast implant so there's absolutely no danger of a thing of salt water you -- because they said that's what we're made up. So the other -- of opponents -- silicon mr. silicone gel implants. Which are little different. With saline. -- state the implants com empty. And we feel the mop during the surgery so we can adjust from -- little bit we can meet once a bigger than other free need to. But because we fill them up there's a valve there's civil -- when we're done -- we pull this little to -- and this little valve seals over. So now like any other mechanical device occasionally. Those valves can fail so you have what's called -- deflation it is kind of like having a flat tire out. In India one dress that still there and on the other -- is kind of flat like Connolly started. But what's happened is that the valve as public given away and sailing has leaked out into the pockets and absorbed by anybody in this it's like drinking a glass of water really. There's no there's a -- about. So in that case you have to go back in and take out the ruptured implant and put it in new implant which is a very simple operation in the pocket sort -- been made. There's really no danger do you there's no pain involved with the very simple procedures -- little bit of a nuisance. Mum. But that's that that's the way it is with silicone gel. The silicone is sort of a sticky thick. Product and they come pre -- so there's no there's no valve so when you put those implants and they are fixed size and -- shape. And they don't really leak eruption anyway and have seen them actually eruption in severe case of -- the car act sooner -- to the test but. In general they don't. We also have an email question says some. Is there a different feel to the and necessarily so different then and other material. Can't I would say definitely yes. Silicone implants feel more natural. There's soft there there are a little bit squishy year you can compress them a little bit differently. The saline is is it hasn't mentioned salt water and -- They're really good they don't compressed quite as well for most people it doesn't make a difference -- May be just a personal.

I think there's a couple things yeah one is if they've got a lot of breast tissue and you can't tell one whether it's a silicone are sailing. But for a lot of thin women. Same -- sometimes you can feel a ripple on the edge of cited the press is sometimes she might even hear a little bit of a sloshing. Sound in in their case will usually recommend a cell economy and com they do. Date you can really pull off looking like you have real -- with the silicone implants.

I'd say in most patients who have implants done. When they take Fisher and a finish area the implants. And you should not be able to tell whether -- punish him.

That's true if you have the right implant the right person I think a lot of people always think about. These hard circles. And that means for there are those people that want that -- via. On but most of the people that we end up scene I think are looking for a very conservative. I just wanna get back what I have lost. That maybe a little bit more so that I can wear some things that I feel mine I'm unable to wear. Outside of that way you know it it's more com practically the women in their forties and fifties that are coming in.

Yet there's several age groups that. Commitment which part of a cut above above Muslim -- muscle because that's and I think you need to also disperse some of those worries that silicone let's get the Sony realized that back out on the mark yet the S and then -- off the -- for but twenty years the FDA didn't extensive study. And thousands thousands of women always every scientific study ever looked at. Has not found there's any harmful effects of the silicon to the back from the market undershirt could use. --

Jealous also changed the matter that it's a cohesive gel and I always kind of described to peoples like egg white when you. -- The whole thing books you know it doesn't separate into all these different parts which is what the original Sonia Gandhi did. Yes but much much it stays together even though it's. July -- the last days as one man -- Com and.

The other guess is Jewish highway is just to -- a group of women from nearly eighteen needed 2528 pregnant have never had any best issue we just want to have something they've never -- they have one big one yeah breasts as different 2000. Then there's a whole group of women who have had. No significant amount Vista should but through aging of pregnancy were -- breast -- breastfeeding have lost what they wanted to reestablish what they had before. Then there's another group of women who. Have had children breast fed are not breast fed and their breasts are now droopy.

I think for winning in Peri menopause where their. Breasts become more fatty yeah is there the gland -- part -- so. In those cases we have to decide whether news of what's called a lift where Europe kind of lifting the best by taking out skin you know to make them the humble -- of them pressed tighter. Boy -- adding more volume to the to the best by adding an imprint or you're doing a little of both Adams and by Indian removing some scanned the kind of tighten the envelope and increased the line. So those are only decision that can be made by guessing the patient in an impatient but. Or in the forties to fifties you know went. Com they've gone to filler is they've gone to both have they've gone through lots of perceived perhaps. And beyond twelve they still have some loose skin on the net -- in the jowl area which is a woman's jaw line. Loose skin along the area between the nose and upper lip that civil -- is live before there. And -- procedure involves. Com separating the skin of your face part of the -- fish from the area below which is muscle. Tightening up -- muscular layer. And then tightening up the extra scanning cutting off the interest can so -- is pulling everything back to the way used to be. And what we've also learned over time is that that isn't enough pulling skin tightening is not enough and if you listen to our last show. New class of surgeons are the -- wanna go to for these kinds of Puget because we have been the best trained to me we've been trained in this stuff and than thousands and thousands procedures. But if you go to a weakened corsets thing your OB GUN doctor or your primary care doctor or your dermatologist or any -- doctors from us -- doctor. And detailing you know not make enough money doing you know picking noses are putting -- balls are living babies are now on make more money. The slept a circus of looks kind of easy I know we can course. With someone who knows about up -- nick and they can teach me how to do it. Now come on Monday morning I'm gonna be elevenths a conductor and they can write -- the expense of their crisis at hand and using I'm getting this. -- down on this happens everyday and have plenty of doctors. Who have really never trained in any surgical procedures that'll plastic surgery. That's -- dollar -- well but you also learn from people who have done it every day for 2030 years and you're you're learning the pearls from people who can teach you. From their mistakes you know so. You're not reproducing the mistakes of everyone you know in a starting -- history all over again.

Well some of what I'm getting from people is thinking that these. On -- offering like a quicker version way there's less down time. You know you hear a lot of these other laser. Liposuction. Machines out there now in you know first off I would assume that only people love the small. Size are really looking for sculpting could even venture test something done in that type of setting. But secondly the problem is is that they their misled to think there's a quick way you know in an out of this procedure and you and I both know at the end of the day the traditional method is you get a certain result no matter how you go about getting there.

I think our listeners probably. That this -- statements probably gonna resonate with minimal listeners if it sounds too good to be true it probably is. And when people tell you all you can have liposuction -- and be back toward tomorrow's numbers NASCAR and a pain not gonna have. If that happens then they didn't do a darn thing I think anytime you have surgery there's a process there is their pain is recovered the swelling and bruising is all kinds of things have gone. As someone promising you all you're gonna benefits of direct back toward tomorrow on the great -- of credibility that from -- and well --

It does have to setting expectations. Now in helping people to learn that. Every procedure has some downtime even just in general healing people are often tired this -- don't have quite the same energy that I. You know it's been a couple weeks in your body spending on its time healing after whatever procedure you happy -- and a minor one.

So I guess that take Thomas who -- gonna switch gears a second title of a -- out of the takeaway message from this segment has been. He wanted us -- who's done a lot of these procedures. Who's got a particular experience who gets good results who's now become afraid to show that there before and after another regularly talk to patients. When you go to see somebody for a one of these procedures just make you feel comfortable with them because you're going to be putting your life in their hands in the sense and entrusting your outcome to them you wanna be sure you're comfortable with at a down. Tell us a little bit about what you do how you do it where you go and and some of the issues you found in and in these places you travel to.

Well thanks very much for that opportunity I work in the -- office Monday Wednesday every other Friday. Primarily helping win the surgical. On layout but also way is treating patients for Botox and filler and then I go out to different spies. In the area on those alternate days I set up my own schedule hands on they line up their clients that are interested in doing. Botox and sellers. In primarily when I see people my first thing is to really sit them -- and get a good sense of of what they're looking for an oftentimes. A lot of times at first -- is really educating them about what IC. As. You know at what they need to think about doing them and that's not a process in itself. And so you.

You what is the most common thing you think you do when you travel to these Botox definitely. And which the most common there in the tree with boats. Between their brows in people on the frown lines and and when people -- how long does this left when you would you tell them.

Ideally Ford -- six months depends on how strong the muscle is and how much we inject.
